Forms & Documents

The two REQUIRED forms for all wilderness medicine courses (NOT required for CPR or basic first aid classes) are our Medical Form and Liability Waiver. All students enrolled in a CWS wilderness medicine course must complete a Release of Liability Form and a Participant Medical Form before they can participate in the class. These forms are located on the Course Downloads Page of your course. Please use the username and password provided to you upon registering for a course, to access that page and access the forms.




An easy way to stay current with the latest in wilderness & remote medicine (as well as other aspects of health, personal well-being, and of course outdoor safety is to read these articles). They're a great refresher and are great for research projects!
